Report: Bridge inspectors raised concerns beginning in 1996

Now it seems to me just by the title of this, you would think that this could have and should have been prevented from happening. If you don't know, a bridge collasped in Minneapolis, Minnesota during the evening rush hour on August 1st. The eight laned bridge fell into the Mississippi River and five people were killed, while eight remained missing. In 2000, some engineers had warned that the bridge needed to be replaced after reports had already been made four years earlier. I can't believe that this has happened, especially since people knew that it needed to be repaired a long time ago. I don't know who should be held responsible but I think someone should. People are dead over something that could have been prevented.
